Access Your Medical Records Online for Peace of Mind
Our medical records are a way for our providers to follow our health and our health care. They can also offer valuable background if you need to see a specialist, or they can help catch a new doctor up on any conditions you have.
Having a copy of your medical records on hand can bring peace of mind if you or a doctor ever needs to access your medical history. The good news is, while we’re practicing social distancing, you can gain access to your records without having to visit your provider in person.
Requesting Your Medical Records from Wayne UNC Health Care
As state and local public health officials continue taking steps to protect health care workers, patients, families and our community, it may be a good time to request your medical records or the records of a loved one online.
If you’re a patient of Wayne UNC or a UNC Physician Network practice, you can request records from September 23, 2018 to present through My UNC Chart.
If you are not signed up on My UNC Chart and would like to use it, please visit www.myuncchart.org and follow the prompts. For additional assistance, please call 1-888-996-2767.
For records prior to September, 2018 or if you don’t have access to My UNC Chart, you can:
- E-mail us - we will reply with a release that can be faxed or scanned to e-mail and returned. The records then can be mailed to your home address or faxed to your physician office.
To retrieve records for patient care, please have your provider fax a request for the medical records to 919-587-2975. We will fax the medical records to your provider.
Protecting Your Medical Records
It’s important to remember that your medical records contain important and confidential information. If you are housing them in your home, take care to ensure they are safe from prying eyes or theft.
By providing a fuller and more accurate picture of you or your loved one’s condition, doctor-provided medical information can help your providers develop a treatment plan that meets your individual needs.
