Your medical records are private and confidential at UNC Health Wayne. Use our simple process to view or get a copy of your medical records.

My UNC Chart – Online Medical Records

My UNC Chart is a free, secure online patient portal that allows you to see parts of your electronic medical record as you receive care from UNC providers. Log in or request an account to see select test results, appointment information, and summaries of your clinic visits and hospital admissions.

How to Request Your Medical Records

To request a complete copy of your medical record from UNC Health Wayne, fill out the appropriate form:

Complete, sign, and return your authorization form by:

Get a Copy of Your Medical Records

If you request your medical records for personal use, copying fees will apply. You’ll get a bill from Scan Stat Copy Service when you receive your medical records.

You have the option to receive your medical records by:

• Mail – Processed within three to five business days of receipt of your completed form (fees apply)
• In-person pick up – Get your records at our health information management department once we notify you they are ready for pick up

Send Your Medical Records to a Provider

We can fax your records to another medical provider for follow-up care the same day we get your request form at no charge. We don’t fax medical records for non-patient care purposes.

Request an Amendment Your Medical Records

You may request an amendment of your protected health information (PHI) with the Request for Correction/Amendment form [PDF]. Complete and submit your form to the health information management department by:

UNC Health Wayne must act on your request for an amendment no later than 60 days after receipt of such a request. If UNC Health Wayne is unable to act on the amendment within the required 60-day time limit, UNC Health Wayne may extend the time for such action by no more than 30 days, provided that:

• A UNC Health Wayne health information manager provides you with a written statement of the reasons for the delay and the date by which action on the request will be completed; and
• UNC Health Wayne may have only one such extension of time for action on a request for an amendment.

You’ll receive a written notice of acceptance or denial of your amendment request.
