Advance Directives

What would happen if you experienced a critical medical problem and couldn’t speak for yourself? Is there someone who knows your wishes concerning your care, life support or other interventions?

An Advance Directive is a legal document that communicates your end-of-life wishes to your loved ones, doctor and healthcare team. This document can save your family a tremendous amount of pain and anguish; and keep in mind, you can change your Advance Directive at any time.

In North Carolina, there are three types of Advance Directives:

  • Living Will
  • Healthcare Power of Attorney
  • Advance Instruction for Mental Health Treatment

Having an Advance Directive is a personal choice. At UNC Health Wayne, we believe the decision process concerning your care is made easier if you have designated someone to speak on your behalf.

Upon admission you will be given a brochure that explains Advance Directives in more detail. However, if you would like more information, please ask your nurse, call 919-731-6407 to speak with Patient Care Coordination or contact us online.
